Mendonoma Health Alliance

Mendonoma Health Alliance is a nonprofit that was formed in 2016 by Santa Rosa Memorial Health, Redwood Coast Medical Services, and Coast Life Support District Ambulance Service. We are located on the North Coast of Sonoma County and our service area covers Irish Beach to Cazadero.

Our MISSION is to improve local access to wellness education, prevention services, and quality healthcare through creative solutions in collaboration with our community.

Our VISION is optimal health and wellness necessary to promote quality of life for everyone in our community.

The CORE VALUES of Mendonoma Health Alliance include person-focused decisions, integrity, collaboration, excellence, accountability and stewardship. These values guide our actions as we work together to provide healthcare services to our community.

We embrace the whole community, convening valuable resources from our partners, and striving to be a "one-stop-shop" on the road to changing the overall health of our community.

Our well-trained Community Health Workers offer many types of services including Care Transitions (hospital to home), Educational Workshops, Chronic Care Management Support Network, Wellness Screenings, Community Outreach & Referrals, Opioid Prevention, Wellness, & Education.
